Step 1: Gather your documents

Before starting the Z83 form, collect your South African ID (book or Smart ID card), certified copies of your qualifications (matric certificate, diplomas, degrees), your CV or resume, and contact details for at least three professional references.

Step 2: Fill in your personal information

Complete Section A of the Z83 with your full name, surname, date of birth, 13-digit South African ID number, gender, race (for employment equity purposes), disability status, citizenship, and contact details including phone number and email address.

In Section B, fill in the reference number of the position you are applying for, the name of the department or institution, the job title, and the province where the position is based. These details are found in the job advertisement.

In Section E, provide at least three professional references. Include each reference's full name, job title, organisation, telephone number, and email address. References should be people who can speak to your work performance and character.

Step 7: Complete the declaration and sign

Read the declaration in Section F carefully. By signing, you confirm that all information provided is true and accurate. Sign the form with your usual signature and add the date in DD/MM/YYYY format. An unsigned form will be rejected.
