UI-19 — UIF Registration Form
Register employees for the Unemployment Insurance Fund.
UI-2.1 — Application for Unemployment Benefits
Apply for UIF unemployment benefits from the Department of Employment and Labour.
UI-2.7 — Remuneration Whilst in Employment
Record remuneration received by the employee while still in employment during leave or reduced working time. Required for UIF claims.
UI-2.8 — Authorisation to Pay Benefits into Banking Account
Authorise UIF to pay unemployment, illness, maternity, or adoption benefits directly into your bank account. Must be signed by your bank.
