What is the difference between "Guardian of a minor child", "Independent child", and "Nominated beneficiary"?

These are the three relationship options on the UI-2.6 form. Choose "Guardian of a minor child" if you are applying on behalf of a child under 18. Do I need the UI-2.8 banking form as well?
