UI-2.7 — Remuneration Whilst in Employment
Fill the UI-2.7 form online free. Your employer declares your salary history for UIF unemployment, maternity, or illness claims. Download completed PDF.
UI-2.8 — Authorisation to Pay Benefits into Banking Account
UI-2.8 — fill the Applicant section (your name and ID) and take it to your bank for the Financial Institution section to be stamped. Required by UIF.
UI-19 — UIF Registration Form

How many forms do I need for a UIF claim?
You need 3 forms: UI-2.1 (your application), UI-2.7 (employer salary declaration — your employer fills most of this), and UI-2.8 (banking details — your bank stamps this). Fill all 3 at once using our toolkit.

Who fills out the UI-2.7 form?
The UI-2.7 requires information from both you and your employer. You can pre-fill your personal details using FillMeIn, then give it to your employer to complete the salary and employment history sections and sign.
Do all UIF claim types use the same forms?
Yes — unemployment, maternity, illness, adoption, parental leave, and dependant benefits all require the same 3 forms (UI-2.1, UI-2.7, UI-2.8). The difference is in the supporting documents you bring.
How long does it take to get my UIF money?
If all your documents are in order, expect your first payment within 6-8 weeks. Benefits are backdated to the date you became unemployed or started leave.
Is the UI-19 form for employees?
No. The UI-19 is for employers to register their employees with the UIF each month. As an employee, you only need the UI-2.1, UI-2.7, and UI-2.8 to claim benefits.
What about the W.Cl.2 form?
The W.Cl.2 is for workplace injury compensation (COIDA) — a completely separate system from UIF. If you were injured at work, your employer must report it using the W.Cl.2.
