How many forms do I need for a UIF claim?

Usually 3: your claim-specific application form (UI-2.1 for unemployment, UI-2.2 for illness, UI-2.3 for maternity, UI-2.4 for adoption, UI-2.5 for a surviving spouse or life partner, UI-2.6 for a child or other dependant, or UI-2.9 for parental leave), UI-2.7 (employer salary declaration — your employer fills most of this), and UI-2.8 (banking details — your bank stamps this). Can I fill all my UIF forms at once?

Who fills out the UI-2.7 form?

The UI-2.7 requires information from both you and your employer. You can pre-fill your personal details using FillMeIn, then give it to your employer to complete the salary and employment history sections and sign.

Do all UIF claim types use the same forms?

No — the application form itself is claim-specific. Unemployment uses UI-2.1; illness uses UI-2.2; maternity uses UI-2.3; adoption uses UI-2.4; a surviving spouse or life partner claiming a deceased contributor's benefits uses UI-2.5; a child or other dependant uses UI-2.6; parental or commissioning parental leave uses UI-2.9 — we support filling all seven online. UI-2.2 and UI-2.3 include a medical certificate section that your doctor must complete on the printed form. Every claim also needs UI-2.7 (employer declaration) and UI-2.8 (banking details) to complete the bundle.

How long does it take to get my UIF money?

If all your documents are in order, expect your first payment within 6-8 weeks. Benefits are backdated to the date you became unemployed or started leave.

Is the UI-19 form for employees?

No. The UI-19 is for employers to register their employees with the UIF each month. As an employee, you need your claim-specific application form (UI-2.1, UI-2.2, UI-2.3, UI-2.4, UI-2.5, UI-2.6, or UI-2.9, depending on your claim type) plus UI-2.7 and UI-2.8 to claim benefits.

What about the W.Cl.2 form?

The W.Cl.2 is for workplace injury compensation (COIDA) — a completely separate system from UIF. If you were injured at work, your employer must report it using the W.Cl.2.
