What is the UI-6A form?

The UI-6A is the "Declaration to Confirm Unemployment Status". It is a continuation form under section 17(4) of the Unemployment Insurance Act (Act 63 of 2001), read with regulation 3. Once your UIF unemployment claim has been approved, benefits are paid in instalments. Before each further payment, you complete a UI-6A to declare that you are still unemployed and have not been paid for any work since you last completed a continuation form.

Who needs to fill in the UI-6A?

The UI-6A is a claimant form — it is completed by the unemployed person who is receiving UIF benefits, not by an employer. If you have an approved UIF unemployment claim (submitted on the UI-2.1) and are still unemployed, you use the UI-6A to confirm your status and continue receiving payments.

How often do I submit the UI-6A?

You submit a UI-6A each time you need to confirm you are still unemployed before your next benefit payment — effectively once per payment cycle for as long as you remain unemployed and are entitled to benefits. Submit it promptly so your payments are not interrupted.

Where do I submit the UI-6A?

Hand the completed and signed form in at your nearest Department of Employment and Labour office (Labour Centre). What do I declare when I sign the UI-6A?

Can I post-date the UI-6A?

No. The UI-6A states clearly that no post-dated forms will be accepted or processed. You must sign and date it with the actual date you complete it.

What must I do if I have started working again?

If you have resumed employment or returned to your normal working hours, you must inform the Department of Labour offices immediately and request your new or current employer to submit a UI-19 declaration. On the UI-6A itself, you enter the date you commenced work, the name of your new employer, and their contact number in section 7. Continuing to claim after resuming work — without declaring it — is an offence.

Do I need to complete the address section every time?

No. The address section is only for a change of address. If your cell number, landline, postal address, postal code, or residential address have not changed since your last claim, you can leave those boxes blank — the UIF already has your details.

What if my banking details have changed?

You confirm your banking details (account holder, bank, branch code, and account number) on the UI-6A yourself — this portion does not need to be completed by the bank. However, if your banking details have changed, the UI-6A specifically notes that form UI-2.8 must also be completed and submitted.

What is the difference between the UI-2.1 and the UI-6A?

The UI-2.1 is the initial application for unemployment benefits — you complete it once when you first claim after losing your job. The UI-6A is the continuation form you complete repeatedly afterwards to confirm you are still unemployed so that each further payment can be released.

What happens if I stop submitting the UI-6A?
